CHAP is a primary care clinic offering medical and dental services to children and adults. We are located in Northwest Pasadena, and we serve all the foothill communities of the San Gabriel Valley and the greater Los Angeles county

CHAP offers all elements of primary care:

* Physical examinations for people of all ages

* Cholesterol checks

* Cancer screening

* Communicable disease screening

* Vision and hearing testing

* Family planning

* Blood pressure screening

* Lab and radiology

* Arrangements for emergency services

* Medications and Prescriptions

* Case management services

CHAP offers dental services for children and adults, and services include routine cleaning, examination and restorative work. The clinic accepts Denti-Cal and other insurance plans. Our friendly bilingual (English and Spanish) staff is here to help you keep a healthy smile!

* Monday 9:00am to 6:00pm

* Tuesday, Wednesday, and Thursday 8:00am to 5:00pm

* Friday 8:00am to 12:30pm

* Dental office is closed for lunch daily from 12:30-1:30pm

The Bill Moore Community Health Clinic is a State licensed Community Clinic which provides free or low cost primary health care clinic to low income and indigent children, adults and seniors. Services include physical examinations, medical treatment, well-baby care, general pediatric care, women's health care, including mammograms and pap smears, immunizations, TB testing, STD testing, WIC exams, health education and case management. Referrals are made for specialty services as appropriate. Needed ancillary services including medications, laboratory services and radiology services are provided. The clinic is part of the State EAPC Program, the CDP (Cancer Detection Program, for breast and cervical cancer screening) and the Family Pact Program. Medical services are provided by physicians and nurse practitioners. Funding for these services is provided by the State Department of Health Services, private foundations, Medi-Cal, private donations and fee for services.

Young & Healthy is a health care program that provides free medical, dental, psychological and case-management services to low-income, uninsured children in the Pasadena Schools, day care centers and homeless shelters in the greater Pasadena community. Health services are provided by volunteer physicians, dentists, therapists, optometrists and other health care professionals. Young & Healthy is based on the conviction that it is the right of every child to have access to quality health care regardless of income levels and that early health care is an investment in the future of the community.
